This form is to be completed for mattress and ensemble base collections. These items need to be booked separately to other hard rubbish items.
PLEASE NOTE: Mattress and ensemble collections are $25 per item. This is a 50% subsidised fee and allows items to be recycled.
An ensemble = $25 and mattress = $25 so both items together will cost $50 for collection.
You will receive an invoice after we receive your booking request. Once payment is received, your booking will be confirmed.
Please allow up to 1-2 weeks booking time, a collection date will be provided to you after we receive your payment. If you do not receive a booking confirmation, please email email@example.com or phone 8347 5111. Items must be placed on kerbside by 6am however no earlier than 24 hours prior to the booked collection day.

Need to cancel or change your booking?
If the booking needs to be cancelled or changed, you must contact East Waste by email firstname.lastname@example.org or phone 8347 5111 no later than 2 full business days prior to your scheduled collection date. If changes or cancellations are made 2 or more days before your booking, then a credit will be issued to use towards a future booking.
If late or no notice is received the booking will count as 1 collection toward the annual allowance and you will forfeit any fees paid.
If your items are not presented for collection by 6am on the day of your scheduled collection, this will still be counted as a collection and you will forfeit any fees paid.
